The activity, which was held from: 07.30 - 12.30, took place at the Chengho Mosque, Jl. Ivory No. 2 Ketabang Genteng Surabaya.

The purpose of implementing this mass vaccine is, firstly, to maintain the health of employees and teachers in the Office of the Ministry of Religious Affairs in the City of Surabaya. Second, the mass vaccine is a form of preparation for each Education Unit in order to face the upcoming New Academic Year, where it is estimated that Learning Activities will be carried out face-to-face or offline. Thus, it is hoped that the learning activity process will run smoothly without any worries about a new covid cluster in the education unit in the Office of the Ministry of Religious Affairs in the City of Surabaya.

In the implementation of the mass vaccine, participants from the Office of the Ministry of Religion of Surabaya City, consist of 166 people from KUA, 51 people from MIN 1, 33 people from MIN 2, 53 people from MTsN 1, 66 people from MTsN 2, 55 people from MTsN 3, 54 people from MTsN 4, 39 people from MAN, 62 people from the madrasah DPK, 85 PAI teachers and 80 employees at the Office of the Ministry of Religion, Surabaya City. Meanwhile, to avoid crowds at this activity, participants of the Mass Vaccine Office of the Ministry of Religion were divided into five sessions: session 1 from 07.30 - 08.30, session 2 at 08.30 - 09.30, session 3 at 09.30 - 10.30 session 4 at 10.30 - 11.30 and session 5 at 11.30-12.30 WIB.

The implementation procedure started with giving the queue number at the entrance to the execution site. After getting the queue number the participants were asked to fill in their identity according to their Identity card and filled in the questionnaire related to their health condition. Participants were also asked to sign a letter stating their willingness to vaccinate. They were allowed not to sign the Declaration Letter if they objected to the vaccination. Furthermore, the participants queued in an orderly manner and sat on the seats that had been provided. The seat distance was arranged in such a way that no participants were close together, according to the physical distance rule. After getting a call by the officer, according to the queue number, the participant was asked to go to table 1 to record their identity. Next she/he had to go table 2 to check the temperature and blood pressure. Then at table 3 the clerk will ask about his/her history of illnesses that had been suffered. If their health met the requirements, participants could go to the Vaccination table where they would get the Covid vaccine from health workers. After receiving the vaccine, they were directed to the next waiting room. Five minutes later, they received a certificate regarding the timing of the second vaccine.

From the monitoring, it was seen that some people outside the Surabaya City Ministry of Religion Office also participated in the activity. This indicates that good coordination between government agencies and institutions and the community will greatly determine the success of this mass vaccination program.
